Here at Family Care Connections we coordinate and provide Home and Community Based Services funded through the Department of Developmental Disabilities for children, teens and adults. Individuals served meet DDD title 19 criteria and have at least one of the four main disabilities of autism, epilepsy, cerebral palsy and/or an Intellectual disability. We provide and coordinate respite care, housekeeping, attendant care and habilitation. We serve all of the Phoenix including the West Valley area (Glendale, Peoria, Avondale, Buckeye, El Mirage, Goodyear, Litchfield, Surprise)
